The incumbent is accountable for the day to day planning and implementation of a broad-based community care coordination, patient navigation and education program provided to patients receiving surgical, medical, in-patient or outpatient healthcare services in Cancer Care. The incumbent provides care coordination, patient navigation and education, and resource referrals regarding patient needs such as, but not limited to, transportation, translation services, support groups, MD referrals, financial and resource referrals to patients/clients and community from diverse cultures.

The incumbent will be responsible for program implementation and evaluation specific to assigned duties;

identifying safety issues and navigational needs within specific communities and individual families and

providing referrals to other community organizations; and reporting and developing positive relationships with

community organizations, project partners, and leaders from different cultures. The incumbent is also responsible for ensuring that the SCCAN non-RN program component is conducted and performed in a manner consistent with the Mission and Philosophy of the Sisters of Mercy.

The incumbent will provide patient navigation for a specific tumor site or cancer for the region, train and manage the peer navigators assigned to that cancer type, manage support groups, participate in fundraising and educational community awareness activities. Track patient data in the 4D system, support and fill in for other navigators that may not be on site, regional education to patients, physicians and staff regarding assigned tumor site. Assist in grant writing, CME programs, research and database data entry for outcome tracking. The cancer coordinator will be an entrepreneurial, enterprising, and inspirational leader who is passionate about excellence in Oncology patient care and aspires to offer the best possible care. Proven ability to work with a team, excellent communication skills, familiarity with clinical trials, ability to provide emotional support and empathy while developing innovative solutions to patient care barriers, ensuring no patient care delays.



Primarily Work Location at Sierra Memorial Hospital in Grass Valley

Minimum Qualifications:

  • Current CA RN licensure
  • Bachelor's Degree in Education, Healthcare, Community Health or a related field or relevant experience, or equivalent combination of education and experience.
  • Minimum 2 years RN experience in Cancer Care and Treatment.
  • Required training or certification in patient navigation or willing to successfully complete training in patient navigation.

Special Skills:

  • Good presentation and computer skills.
  • Experience in educating and/or navigating adults one-on-one and in larger
  • Comfortable in dealing with death and dying.
  • Ability to deal with people in extremely stressful situations.

  • Masters degree
