Responsibilities

Job Summary:

The incumbent is accountable for all of the supervisory, administrative and operational functions of the Care Coordination Remote Services (CCRS). This working Supervisor position oversees compliance with Medicare 1 Day Stay audits; Serves as a liaison between CCRS and participating CommonSpirit Health facilities, nursing leadership or their designee, medical staff, physicians and acute care staff. The incumbent facilitates performance improvement activities, monitors competency of all staff and ensures timely reviews are completed for the CCRS staff.

Essential Key Job Responsibilities:

  • Under the direction of the System Director of Operations, oversees and supervises daily operations of CCRS.
  • Provides administrative and Leadership support for CCRS and department staff.
  • Collaborates and streamlines relationship and CCRS process with participating hospitals, hospital staff and CCRS department staff
  • Collects objective/subjective data for CCRS program improvement.
  • Collaborates to identify and implement cost of healthcare savings opportunities.
  • Promotes effective communication and relationships.
  • Participates in activities that promote professional growth and development and maintains current knowledge base in all areas of responsibility.
  • Keeps abreast of current legal, accreditation, and regulatory requirements and applying into relevant standards and appropriate policies and procedures for CCRS and licensed staff.
  • Maintains channels of communication between and among entities throughout the network of participating CCRS hospitals, which encourages interdisciplinary cooperation, respect, and support CCRS operations and operations of any new related CCRS program.
  • Interacts with and maintains a supportive role with hospital leadership, Hospital Case Management, Hospital Medical staff and Hospital Emergency Department staff.
  • Supports the System Director in the management of the CCRS overall operations.
  • Seeks input from and provides education to CCRS staff and participating hospitals.
  • This supervisor would need to spend 30% of time performing Medicare 1 Day Stay audits.

Qualifications

Minimum Qualifications:

Education and Experience:

  • BSN Preferred
  • Bachelor's degree and min. of 1 year of experience in discipline OR 3 years' experience in discipline OR Master's Degree and no experience required.
  • Strong clinical background

Licensure and Certifications:

  • Current RN License

Knowledge, Skills, Abilities and Training:

  • Strong clinical background and knowledge
  • Clinical process redesign experience, Process improvement and mapping skills, knowledge of Managed Care, Population Management and Utilization Management
  • Principles of integrated healthcare, basic financial analysis/spreadsheet skills
  • CMS, DMHC, and JCAHO, state and federal regulations affecting population, utilization, and case management
  • Shall comply with all applicable laws and rules including the Common Spirit Health Standards of Conduct and policies and procedures that apply to his/her job responsibilities, seek guidance when in doubt, promptly report any potential or suspected violation of Standards of Conduct, policy or procedure or applicable laws/regulations and when requested, assist staff in investigating all allegations of violation.
  • When called upon, shall assist in risk evaluation and work collaboratively with facility/entity leaders to implement, monitor and mitigate risk corrective plans outlined by the Compliance program.

Incumbent will maintain appropriate records and documentation pertinent to the client/patient/staff and department operation applicable to his/her role. Incumbent will complete all mandatory education on time, and will cooperate in investigation matters as requested.
