Our staff is well-trained and highly skilled qualities that are vital to maintaining excellence in care and service.**Responsibilities****Position Summary:**As part of the Utilization Mgmt team, the SNF Liaison LVN assesses, coordinates, and communicates discharge planning to Mercy Medical Group (MMG) patients. In addition, this role provides Physician support. This role monitors the patients in Subacute Nursing Facilities and out of network admissions using InterQual and makes assessments regarding appropriate lengths of stay. The SNF Liaison LVN coordinates and promotes an appropriate post-hospitalization treatment plan and evaluates effectiveness. The SNF Liaison LVN interacts with multiple disciplines to assure high quality, cost efficient care utilizing available resources for MMG members.**Qualifications****Minimum Qualifications:**- Five (5) years Subacute or Nursing Facility experience including discharge planning, case management, and/or utilization management. Direct experience working with physicians required.- Graduate from an accredited school of nursing, with clear and current CA Licensed Vocational Nurse (LVN) License.- Competent in a variety of computer programs including Microsoft Office suite of products.- Strong verbal, written, and interpersonal skills.- Strong organizational and time management skills.- Excellent documentation skills.- Understanding of assessment tools such as InterQual and DRG's; skilled at interviewing techniques; broad knowledge of medical/surgical conditions in all age groups.**Preferred Qualifications:**- Preferred seven (7) years of acute/Subacute experience in combination of utilization management, case management and discharge planning.- Prefer working knowledge of InterQual.\#HelloCareCoordination**Equal Opportunity**CommonSpirit Health--- is an Equal Opportunity/Affirmative Action employer committed toa diverse and inclusive workforce.
