Some benefits may not be applicable to on call or part time employees.JOB SUMMARYThe LVN organizes and implements nursing care for an assigned group of patients. The LVN assists the RN in planning patient care. The LVN functions as a member of the nursing team and performs established nursing procedures and related duties as required.ESSENTIAL JOB FUNCTIONSPrimary Clinical ResponsibilitiesA. Observes and collects data regarding the physical and emotional condition of assigned patients.B. Documents data and observations on graphic and flow sheets in a timely manner.C. Promptly informs Supervising RN and MD of any changes in assigned patients' conditions.D. Reviews the care plan and assists the supervising RN in planning patient care for assigned patients.E. Provides direct nursing care in a safe, therapeutic, and efficient manner and in accordance with established policy and procedure.F. Carries out physicians' orders in accordance with plan of care.G. Participates in patient care conferences.Communication/Interpersonal RelationsA. Interacts with patients and families in a manner they perceive as positive and supportive.B. Communicates appropriately and effectively with all members of the healthcare team.C. Facilitates problem solving, utilizing resource persons as needed.D. Assists RN with patient and family teaching which is outlined in and consistent with the Licensed Vocational Nurse Practice Act.E. Assists with orienting new staff members and student nurses to the role of LVN.QUALIFICATIONSEducationValid California Licensed Vocational Nursing license. Current CPR Certification.Training and ExperienceOne year recent home health nursing preferred. One year in acute care setting.Able to learn to operate a laptop computer to perform clinical documentation and keyboarding or typing skills; previous experience preferred.Knowledge and AbilitiesPossesses an in-depth knowledge of comprehensive nursing care which includes: Clinical disease entities common to assigned home care patients; the physical assessmentskills required for appraisal on all assigned patients; the treatments, diets, and medication regime for individual cases; the rehabilitative and preventative aspects concerning each patient; additional direct patient care service which ensures the safety, comfort, and protection of patients; the usual performances indicated for disease prevention and restorative measures; ability to document professional nursing services provided with a realistic plan for reaching short term and long range goals; able to make decisions regarding safety need of patients' home including adequate equipment.OtherPossession of a valid California driver's license, access to an operating automobile, and at least minimum coverage car insurance.The final candidate must successfully pass Eskaton's post offer, pre-employment testing which includes a criminal background check, drug test, COVID test, TB screen test and health screen.Eskaton is an Equal Opportunity employer.
