You can also follow us on Twitter @DignityHlthSac and Responsibilities Position Summary: This position oversees the 'Healthier Living' Education Program and is formally titled as the Community Benefit Program Manager. The program specifically targets uninsured and underinsured (Medicaid, Medicare) populations. This position will lead community health education classes, creating virtual forums to educate our community about healthier living options. The 'Healthier Living' Education Program uses two evidence-based educational workshop models: - CDSMP, Chronic Disease Self-Management Program - DEEP, Diabetes Empoowerment Education Program This position is required to become certified as an educator in both workshop programs and facilitate workshops. This position supports the initial program implementation, and will oversee on-going program efforts. Due to COVID-19 this title will be a remote title for the foreseable future as we transition the 'Healthier Living' Education Program into a virtual learning space. Once its safe to do so, this position will return to in person program facilitation. This position is a part time/ 20 hour per week, primarily day shift (but schedule is flexible.) Qualifications Minimum Qualifications: - Bachelor's degree - Three (3) to five (5) years of related experience to include program management, program planning, training/facilitation, and program evaluation. - This position is required to complete training for CDSMP and DEEP education programs (this training will be sponsored by Dignity Health.) - Special skills required include: - Knowledge of the Sacramento region; which includes Sacramento, Yolo and Nevada counties. - Computer proficiency in common office applications and the virtual meeting rooms. - Knowledge of community health and wellness programming.
