Management **Overview** Sierra Nevada Memorial Hospital has proudly
served as the sole healthcare provider for western Nevada County
for more than 50 years. Much has happened since the hospital opened
in 1958, including additions in 1994 of a 68,000 square foot
Outpatient Center, a comprehensive Cancer Center and most recently,
the opening of the Sierra Nevada Diagnostic Center in the fall of
2006. SNMH is a licensed, acute care hospital providing a full
range of services including cardiovascular, surgical, emergency and
diagnostic. SNMH offers access to the SNMH Health Sciences Library.
The library features continuing and medical education for
healthcare professionals, diabetes classes, prenatal classes,
cancer support groups, and family caregiver classes. In addition,
we have more than 90 board certified primary and specialty care
physicians on our active and associate medical staff.
**Responsibilities** Join our team as the Manager of Home Health
and become an integral part of Dignity Health Home Health Agency
supporting our Nevada County market. The position manages the home
health service line that contributes collectively to the provision
of high quality patient care. In this leadership role you will
serve as a valuable resource as the Director of Patient Care
Services (DPCS), monitoring and supporting the desired outcomes as
established for the home health agency. **Core Duties** : +
Collaborate throughteam building with multiple stakeholders such
as, areasupervisors and directors as required to support the
hospital's role as a community health service provider + Assists in
developing and implementing a unit Quality Management Program
consistent with hospital wide Performance Improvement Plan. Using
CQI principles, operates the units within the financial and
productivity guidelines setting goals and developing policies that
support this objective. + Provides for quality services by ensuring
that the staff are well trained in the services to be provided. +
Addresses and resolves agency issues, including problem resolution
related to customer complaints, concerns and suggestions. **$5,000

**Qualifications** **Minimum Qualifications:** + Registered Nursing
License (RN), or other clinical specialties (e.g., Occupational
Therapist, Physical Therapist, Speech Therapist etc.), licensed in
the state of practice + Associate's degree in Nursing or
healthcare-related field or business, or three (3) years of related
job or industry experience in lieu of degree + Minimum of three (3)
years of health service administration management experience
(within the last five years) in home care or a related health care
program. + Other credentials or credential modules may be required
+ AmericanBasic Life Support (BLS) the American Heart Association
(AHA) + Principles and practices of effective management +
Budgetary and planning control + Title 22, COP, The Joint
Commission knowledge as it pertains to home health electronic
